My Care, My Way, Home First Phase 2

In order to remain transparent, this section of the website reflects the work undertaken from November to December 2016.  If you want to find out what has happened since that time, click on 'Menu' at the top of the page.

We recognise that there is significant concern within the population of Stoke-on-Trent and Northern Staffordshire about the capacity available for community-based services.

We are keen to discuss this with people and consider any suggestions about the future shape of services.

To make sure that all the necessary healthcare services are available and to keep people in their own homes wherever possible we need to change the way we have been doing things. No final decisions have been made but we know that we simply cannot continue as we are.

Feedback from the engagement activity during November-December 2016

During November and December 2016, the CCGs engaged with the public on the proposals for the My Care, My Way – Home First model of care.

The engagement included an online survey, public events, engagement with key stakeholders and information provided through many channels so that people could let us know how they felt, with a particular focus on the provision of Adult Intermediate Rehabilitation Services provided from the Community Hospitals.

The feedback from the engagement will be given due regard and will be used to inform future consultation on the future of Community Hospitals.

The engagement process was assured by NHS England and was informed by previous formal consultation on My Care My Way – Home First.

We are grateful to everyone who took the time to express their views.

The feedback we received has been summarised in the documents below:
